Thank you for having an interest in hiring me as a performer for your big event. This form is the first step in the process, and by filling it out and submitting it, you will be providing me with the information I need to give you an initial quote for my services. If you are satisfied with this quote, then I will forward a contract to you, which must be signed and returned to me before I can give you a final confirmation.

Please be sure to fill in all applicable fields to avoid any delays in getting your inital quote. Whenever possible, I will try to respond to requests within one week.
